Njernda operates two separate programs for people living at home and to help maximise their independence.
CHSP – The Commonwealth Home Support Program, which provides entry-level support for older people who need some help to stay at home.
HACC PYP – The Home and Community Care Program for Younger People provides entry level support for these people who are living at home to help maximise their independence.
Are you eligible to Access Njerndas’ HACC PYP?
Njerndas’ Home And Community Care Program for Younger People (HACC PYP) target group is Aboriginal or Torres Strait Islander people aged under 50 years old, who do NOT have an aged care or disability support package and who need help with regular activities to be able to better manage living at home.

It is OK to have someone you know and trust be with you and support you during the In Home Assessment.
If you would like this person to also be your contact person, ask the person making the referral to make a note about this on the referral.

Njernda’s Commonwealth Home Support Program (CHSP) will provide entry level support for frail, older people who are living at home to help maximise their independence.
CHSP services can improve your well being and help you stay independent.
The CHSP is not designed for older people with intensive, multiple and complex needs. These higher needs are generally supported through other aged care supports including the Home Care Packages Program, residential aged care and through the health care system.

Would you like help along the pathway to accessing CHSP services?
It is OK to have someone you know and trust be with you and support you for the In Home Assessment and to speak for you on the telephone to the My Aged Care operator.
You will need to speak to the operator to identify who you are and to give verbal consent for someone else to speak for you.
